The WP9 pilot study tests a personalised, multilevel implementation package to accelerate the integration of digital survivorship care across diverse clinical sites in EU Member States. Although digital oncology solutions such as remote monitoring, supportive care tools, and patient self‑management platforms are rapidly expanding, real‑world implementation lags, particularly outside high‑resource settings. Barriers include variation in organisational readiness, skills gaps, resistance to change, and limited digital infrastructure.

At least eight sites will participate: four low‑readiness, two middle‑readiness, and two high‑readiness sites. Sites will be stratified based on existing survivorship care capacity and digital health maturity, and the implementation package will be tailored accordingly.

The pilot will evaluate whether the approach:

  • Improves readiness in low‑readiness sites;
  • Enables successful deployment and evaluation of digital tools in routine care in middle‑readiness sites; and
  • Enhances equity and sustainability of digital health pathways in high‑readiness sites.
